Welcome to Providence Dental Practice ! By using our services, you agree to these Terms and Conditions. Please read them carefully. If you do not agree to these terms, you should not use our services.
1. General Information
- Providence Dental Practice is a professional dental clinic offering a range of dental services, including but not limited to preventive care, restorative treatments, cosmetic dentistry, and orthodontics.
- These terms apply to all patients, visitors, and users of our services.
2. Appointments and Scheduling
- Appointments can be scheduled via phone, email, or online booking system on our website.
- We kindly ask that you arrive at least 10 minutes before your scheduled appointment to complete any necessary paperwork.
- If you arrive late, we may not be able to accommodate your treatment, and a rescheduling fee may apply.
3. Payments and Fees
- Payment for services is due on the day of treatment, unless otherwise agreed upon in advance.
- We accept various payment methods, including credit/debit cards, bank transfers, and cash.
- If you are covered by dental insurance, we will assist with the claims process, but any balance not covered by insurance is your responsibility.
4. Returns and Refunds Policy
- Treatment and Services: Due to the nature of dental services, all treatments, procedures, and consultations are non-refundable once performed.
- Dental Products: If you purchase dental products (e.g., toothbrushes, teeth whitening products, mouthguards), we will accept returns only if the product is unopened, unused, and in its original packaging, within 14 days of purchase.
- Defective Products: If a product is found to be defective or damaged upon receipt, we will provide a full refund or exchange upon verification of the defect.
- Cancellations and Changes to Appointments: If you need to cancel or reschedule an appointment, we request that you notify us at least 24 hours in advance. Cancellations made less than 24 hours before the appointment may incur a cancellation fee.
5. Cancellation Policy
- Appointment Cancellations:
- If you need to cancel or reschedule your appointment, please notify us at least 24 hours priorto the scheduled time to avoid a cancellation fee.
- Cancellations made within 24 hours of the appointment or failure to attend will incur a cancellation fee of £50.
- Procedure Cancellations:
- In the event of needing to cancel a scheduled procedure, we ask that you notify us 48 hours in advance. We may require a deposit for future appointments, which will be non-refundable if you cancel within 48 hours.
- Refunds for pre-paid treatments will only be granted under exceptional circumstances and at the discretion of the clinic management.
- No-Show Policy:
- Failure to show up for a scheduled appointment without prior notice will incur a no-show feeof £50.
- Repeat no-shows may result in being required to pre-pay for future appointments.
6. Patient Responsibilities
- It is your responsibility to provide accurate and up-to-date health information, including any allergies, current medications, and medical conditions, before receiving treatment.
- Failure to disclose this information may result in delayed or unsuitable treatment.
7. Privacy and Confidentiality
- We are committed to protecting your privacy and maintaining the confidentiality of your personal information. We will only use your personal data to provide the services you request and will not share it with third parties without your consent, except as required by law.
8. Liability
- While we strive to provide the highest standard of care, we cannot guarantee outcomes of dental procedures. Each patient’s result may vary depending on individual factors.
- The clinic is not liable for any damage or injury caused as a result of non-compliance with post-treatment instructions or failure to attend follow-up appointments.
9. Changes to Terms and Conditions
- We reserve the right to modify these Terms and Conditions at any time. Any changes will be communicated to you via email or posted on our website. Your continued use of our services after such changes will signify your acceptance of the updated terms.
10. Governing Law
- These Terms and Conditions are governed by the laws of England. Any disputes will be resolved under the jurisdiction of England.
